Sacks are objects that may contain other pickups, in a similar vein to chests. They usually grant less rewards than chests, but do not require keys or bombs to open them.

Fiend Folio adds 3 new kinds of sacks to the game.

Blood Sack

Blood Sack

Blood Sacks are sacks that are used to heal. They can occasionally appear in Sacrifice Rooms or next to other places to donate health, such as Blood Donation Machines. Random sacks have a 12.5% chance to become a Blood Sack.

When opened, spawns 1-3 random Heart pickups that can be any of the following:

• ~36% chance for a Half Red Heart
• ~30% chance for a Red Heart
• ~15% chance for a Double Red Heart
• ~11% chance for a Scared Heart
• ~4% chance for a Blended Heart
• ~4% chance for a Bone Heart

Trash Bag

Trash Bag

Trash Bags are sacks that spawn temporary familiars, sometimes along with a "trashy" pickup. Random sacks have a 12.5% chance to become a Trash Bag.

When opened, Trash Bags spawn the following:

• 6 Blue Flies, 4 Blue Spiders or 4 Blue Skuzzes.
• 33% chance for a Half Red Heart, Half Soul Heart, Blended Heart, Bone Heart, or Rotten Heart.
• 8.33% chance for 1 random object or a 16.67% chance for a worm trinket, but not both.

    • Emergency Contact
    • Glass D6
    • Glass D4
    • Glass D8
    • Glass D100
    • Glass D10
    • Glass D20
    • Glass D12
    • Glass Spindown
    • Glass Azurite Spindown

  • The worm trinket that can be spawned is as follows:
    • Pulse Worm
    • Wiggle Worm
    • Ring Worm
    • Flat Worm
    • Hook Worm
    • Whip Worm
    • Rainbow Worm
    • Tape Worm

    • Ouroboros Worm
    • Brain Worm

52 Deck

52 Deck

52 Decks are rare sacks that drop cards. They are most often found in Arcades. Random sacks have a 5% chance to become a 52 Deck.

When opened, 52 Decks spawn 2-5 random Playing Cards of the following:

• Jack of Diamonds
• Jack of Clubs
• Jack of Spades
• Jack of Hearts
• 3 of Diamonds
• 3 of Clubs
• 3 of Spades
• 3 of Hearts
• Queen of Diamonds
• Queen of Clubs
